We are getting our lawn application division set up at our Garden Center. I was looking for a form that is used in the industry that for record keeping. Our state, Kansas requires that we keep records for each application. I don't think that it would be too hard to come up with something but if I could not reinvent the wheel it would help. I tried Nebs business forms but did not find anything. Thanks

Doesn't your state provide you with a sample form?
In WA, we have at least 3 formates we can use and they are provided by the Dept of Ag which administers the main functions. I have taken one of the formates and modified it for my record keeping requirements.
If they don't provide one, go to Gempler's www.gemplers.com
They have some that may work for you.
